Do not accept this application showing any erasures, whiteout, alterations or mutilations. Instructions 1. Every owner of the manufactured/mobile home must fully complete and sign this affidavit. Any missing information will render the affidavit as incomplete and the title for the manufactured/mobile home cannot be eliminated. 2. Signature of the agent acting on behalf of the lien holder must be notarized. If there is more than one lien holder, an additional affidavit (TR-63) must be completed for each lien holder. Every owner on the manufactured/mobile home must sign all affidavits. The following documents must be attached to this affidavit when submitted to Titles and Registrations for approval: a) Kansas paper title showing the owner(s) and if applicable, the lien holder’s name(s) of the front, or a Kansas E-Title documentation, Ø If the owner of the manufactured/mobile home does not have a Kansas title, the owner may complete an Affidavit (TR-90 form) if the home is 1979 or older and follow the requirements listed on the form. Submit the form and supporting documentation and make an application for a Kansas title at the owner’s local county treasurer’s office. b) A release of lien from the lien holder shown on the front of this form and; c) Copy of most recent receipts for personal property tax and real estate taxes as proof of payment. 3. A signature is required by a county official stating whether the manufactured/mobile home has a permanently affixed foundation. Contact the county where the home is affixed to determine which office has authority to sign. 4. Attach the $10 fee and all required documents and send to: Kansas Department of Revenue Division of Vehicles P.O. Box 2505 Topeka, Kansas 66601-2505 Make your check or money order out to Kansas Department of Revenue. Do not send cash. 4. If you have any questions, contact Titles and Registrations by calling: 785-296-3621, press 3, then press 2. 5. Only original signatures will be accepted. Faxed, copied or e-mailed signatures will be rejected by the County Register of Deeds. COUNTY REGISTER OF DEEDS • The County Register of Deeds may require additional information and/or fees.
